Compose rules

Rules compose as an AND condition: every rule must allow the hit.

Rule	Meaning
{ max: 3, window: '7d' }	At most three hits in any rolling seven days
{ max: 1, every: 'day' }	At most one hit per local calendar day
{ cooldown: '4h' }	At least four hours between hits
{ max: 5 }	At most five hits over the record lifetime

Calendar rules support day, week, month, and year, plus a business-day cutoff and weekStartsOn.

remaining is the strictest count-based allowance after the operation. It is Infinity when only non-counting rules such as cooldown apply. deniedBy is the same rule object supplied by the caller.
